Lord Inquisitor, the Emperor Protects. As per your instructions I have travelled to the Piraxis system in order to complete a profile of the Tycho Mutiny survivors and look for any evidence of Xenos involvement.
The rescue ships are treating the 17 surviving members of the Legio III unit with the highest regard. They are being treated as heroes and it is likely they may receive some commendation for their effort. Personally I can hardly see surviving as a valorous act, however the morale of the military is for the Commissariat to consider not myself. I did a complete scan of every survivor, a thorough work up and psychological profile. All data is included however the summary of the results is that all 17 survivors are stable, safe and free from any warp influence. Octavius Titus Fidus is the only survivor with any mental issues, I believe he has been deeply unsettled by the incident and will never recover. However there is no taint.
The incident seemed to spark from an artefact that Rogue Trader Tycho (declared Hereticus Post Mortem) had purchased from illicit traders. Somehow it was triggered and drove two thirds of the crew insane. That two thirds then proceeded to set upon the remaining third that had been unaffected by incident. From examining the survivors and the crew manifests I have come to the conclusion that the ones that went berserk were of weak will. Tycho was killed by his first officer who was in turn torn to pieces by other maddened crew. Optio Gaia Lecrutia Vettia did lead a solid defence of the stations 3rd and 4th levels against the mad crew but only managed to save herself and 16 of her troop.
Inquisitor Delvay did not pass the medical examination. It was clear that some Xenos taint had infiltrated his brain and burrowed through his cortex. Though whatever caused the damage is no longer present its spoor remained and the Inquisitor appeared to be mentally deficient. My medical team believes that whatever it was either rejected by the Inquisitor’s brain or else something forced it out. After logging my findings I put down the Inquisitor and destroyed the remains.
Magos Thuulian was the only other survivor, Adeptus Mechanicus specialisation Biologus. I interrogated the magos and he revealed a connection with the Holy Ordos dating back centuries to Inquisitor Dallenter (may the Emperor Bless his Soul). He has continued research under instructions of Dallenter for the last 150 years. The research is surrounding what Thuulian believes is the inevitable return of a Xenos race that caused significant upheaval in the Ultima Segmentum in 745M41. He was tasked with searching for new weapons and mechanisms to counter any threat. As Dallenter was a Lord Inquisitor when he died I decided not to press Thuulian for details of his projects. I await instruction from you Lord Inquisitor if you wish me to proceed on this.
The xenos device was destroyed utterly when it triggered the incident. I have personally inspected the remains and it is utterly destroyed. I have had the pieces loaded into a shuttle and driven them into the local star.
I had intended on sending the entire ship into the star but was overruled by Inquisitor Herrod. I am concerned that whatever affected Delvay’s mind is still present though my multiple searches of the ship top to bottom have yielded nothing. Inquisitor Herrod claims I am paranoid and the danger is past. As his junior I have followed his request and signed over the ship for refitting and sanctioned the sale of the charter.

Inquisitor Aldermann’s Personal Log. Dated 15 years after the Tycho Mutiny.
Inquisitor Masters and I have successfully completed the research into the heresy of Kalthos. Whatever entity caused the spawning there and forced the subsequent Exterminatus order those centuries ago we believe is now at work on Naevus. I fear it is linked with the mutiny on board the Tycho’s Endeavour but I am unable to prove it. Worse yet I fear my personal failure to destroy that ship thoroughly while it was in my power has allowed this horror to escape the Emperors Wrath. An error I will never make again. Remember the teachings of Cardinal Gravial “Leniency is a sign of weakness, mercy is a sign of weakness and to negotiate is to surrender.”

I have successfully debriefed Drenis Gaius Pius. He was approached by Archos of the coterie but refused to join. However I believe I moved too openly and have alerted the cabal as Pius was murdered yesterday, no doubt for communicating with me. The information gleaned is valuable but I fear my mistake has put our efforts back by some margin. Pius claimed that Invidius had contact with Inquisitor Delvay before the explosion on the Endeavour, he testified that he saw them together more than once. Whatever the guiding hand behind the Coterie is it has once again eluded me. I am inclined to agree with you and the suggestion you made last time we met. We should both approach the issue from separate sides. We will see if you can flush them into my net and between us we can force the power behind the coterie to reveal itself.
On an additional note, it was confirmed that captured agents could have their minds read, even close after death. I intend to recruit some new agents as I fear my current ones know too much. May the Emperor protect us all.
